Family First Health Nurse Care Manager in YORK, Pennsylvania

We are looking for a dedicated Behavioral Health Nurse Care Manager to join our team and play a pivotal role in coordinating and improving the mental health outcomes of our patients. In this position, you will exercise a great deal of autonomy in working closely with our Psychiatric provider and interdisciplinary teams to assess patient needs, develop care plans, and coordinate ongoing care and support services. Your compassion, clinical expertise, and advocacy skills will contribute to enhancing the well-being and quality of life for our patients.

 

  • Support the mental and physical health care of patients on an assigned patient caseload
  • Participate in regular caseload consultation with the psychiatrist and communicate resulting treatment recommendations to the patient's medical provider
  • Accurately obtains and electronically records all vital information, medication request, allergies and chief complaint, etc. following the rooming of a patient for a visit
  • Prepares exam room and patient for virtual psychiatry visit, ensuring that the provider is able to conduct the visit immediately upon connection via videohealth platform
  • Collects, labels and documents samples for laboratory testing following proper laboratory protocols and procedures
  • Provides medical care management and follow-up to psychiatry patients in accordance with guidelines established by Family First Health
  • Support psychotropic medication management as prescribed by medical providers, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment.
  • Works collaboratively with the Behavioral Health and Medical teams to ensure excellent service and best course of care
  • Facilitate treatment plan changes for patients who are not improving as expected in consultation with the medical and psychiatric provider and who may need more intensive or more specialized mental health care
  • Performs all other duties as assigned
