IGHL- Independent Group Home Living Program, Inc. Day Habilitation Specialists in Yaphank, New York

The expected salary range for this position is between $16.50 and $17.25 per hour. The actual compensation will be determined based on the site, hours/day of shift and other factors permitted by law. We have locations throughout Suffolk County, including Aquebogue, Hampton Bays, and Center Moriches. The shift is Monday - Friday 8:30a - 4:30p. $1,000 SIGN-ON BONUS! TITLE: Day Habilitation Specialist SUPERVISOR: Coordinator of Day Habilitation Services The Day Habilitation Specialist is responsible for assisting consumers with their daily programmatic responsibilities and providing the consumers with training that supports independent living skills. RESPONSIBILITIES: E- Connotes Essential Functions E-1 Receives and completes assignments as directed by the Coordinator of Opts Day Hab Services. E-2 Provides direct supervision of assigned caseload. E-3 Assists the program participants, Service Coordinator and advocate in creating an individualized service. E-4 Attends meetings, completes Day Hab plans, documentation records, PPOs, monthly notes, daily summaries and any other documentation required. E-5 Provides the program participants with the necessary training in the area of maintenance and cleanliness of the facilitys work areas and the safe use and care of facilitys equipment. E-6 Provides the program participants with the necessary training, support and opportunities to achieve outcomes such as: A. Increase sense of self-esteem. B. Increase relationships with others. C. Exercise of choice. D. Development of new skills. E. Improvement of existing skills. F. Community participation and integration. G. Personal enrichment. H. Productive use of time. E-7 Adheres to OPWDD and IGHL policy, including Safety and Code of Conduct; complete agency forms as required: time off requests, tuition reimbursement, physical complaint, non-reportable and incident reports; utilizes time system correctly. 8- Enforces and adheres to all safety policies and procedures. 9- All other duties as assigned. ADDENDUM FOR EMPLOYEES TRANSPORTING CONSUMERS E-1 Responsible for transporting consumers to and from programs and/or daily activities. REQUIREMENTS A. High School Diploma/GED. B. One year experience working with the disabled population. C. Valid NYS drivers license. D. Complete and maintain AMAP certification. E. Experience or interest in instructing the developmentally disabled during real life experiences. F. Must be able to lift 50 pounds.
