Job Description: Job Title: Direct Support Professional (DSP) Program: Supports and Services FLSA Status: Non-exempt Hours: Varies FTE: Varies GENERAL DESCRIPTION: Under the direction of the Site Supervisor this position is responsible for maintaining an environment which encourages and facilitates optimal personal development and provides person directed services while ensuring the health and safety of the individuals served. ESSENTIAL FUNCTIONS: Regulatory and Corporate Compliance Ensures compliance with regulations, agency policies and procedures Ensures required documentation is complete, accurate and legible Ensures the agencys incident management process is followed Provides input in the development and revision of policies, procedures and agency forms as needed Ensures Chain of Command is followed when necessary and instructions are followed as stated in policies and procedures Financial Management Assists with ensuring adherence to the annual budget Assists with ensuring accurate management of individuals funds Completes personal allowance documentation and other financial documentation according to procedure Person Centered Services Provides services to individuals as planned in their Life Plan which includes the persons Individual Protective Oversight Plan (IPOP), Staff Action Plan (SAP), Support Plans, and/or other safeguards and ensures all related documentation is completed contemporaneously Implements activities as outlined in the SAP/Habilitation Plan and, as applicable, the Life Plan Plans and coordinates daily activities Acts as an advocate for individuals Provides person directed services, including pre-voc, employment, community, residential or day habilitative services and assistance in personal skill areas Assists and supports individuals with community integration Respects the individuals right to privacy and confidentiality by appropriate use of written and verbal communication Works collaboratively with others to ensure the individuals health and wellbeing Ensures emergency medical procedures are followed and consults with the medical chain of command when necessary Provides services and follows individuals Plan of Nursing Services (PONS) Reports and documents all individual medical concerns to the appropriate personnel Transports and attends health and medical appointments as assigned Maintains on-going communication with all members of the individuals team Program/Service Management Maintains an environment that encourages and facilitates optimal personal development Completes assigned daily responsibilities Completes specific duties as assigned, if applicable Completes documentation, filing and general office maintenance and other routine operational reports and any other paperwork as assigned Assists in the identification and development of new services and support opportunities that provide quality-of-life choices Physical Plant and Safety Ensures the site is a safe, clean environment and immediately addresses areas of concern Maintains knowledge of fire evacuation plan and ensures the plan is followed Practices standard precautions and other appropriate infection control practices Ensures site vehicles are maintained, and completes vehicle inspections as assigned Contacts with Others Develops and maintains positive relationships both internal and external. This includes family, local businesses, and external service providers Maintains effective working relationships with service providers, community resources and families Ensures that internal and external written and verbal communications regarding the agency are positive and reflect our mission, vision and values and respects the individuals right to privacy and confidentiality Ensures all concerns are addressed in a timely and professional manner
