We are currently hiring a Case Manager - Troy , to work in our Housing Office. ? As the Case Manager , you would be responsible for: ?

  • Facilitate the transition of homeless persons who enter the program and assist them in maintaining permanent housing and support services

  • Perform assessments

  • Develop and implement Individualized Service Plans

  • Provide support and supervision to residents during evening hours

  • Organizes and facilitates support groups and social and recreation activities

    Case Manager Minimum Salary: $18.00 per hour

    Case Manager Maximum Salary: $20.00 per hour

    Case Manager Schedule: Monday – Friday, 2:00pm – 10:00pm

    Qualifications:

  • Associates Degree (preferred)

  • 1-3 years of experience

  • Valid Drivers License

  • Experience working with people experiencing homelessness

  • Experience working with diverse populations
