Stony Brook Medicine (Stony Brook University Hospital) Case Manager Supervisor in Stony Brook, New York

Position Summary

At Stony Brook Medicine, a Case Manager Supervisor is a valuable member of our team, who provides clinical services to our patient population.

Duties of a Case Manager Supervisor in the Care Management Department may include the following but are not limited to:


  • Support efficient use of resources.

  • Oversee staff performance including new staff orientation
  • Knowledge of Utilization Review and LOS

  • Review of high risk complex cases.

  • Input into transitional care

  • Oversight of department systems and process

  • Provide direction and guidance to staff for roles and responsibility..

  • Education of staff with new processes

  • Input in performance on other disciplines within the department.

  • Provides input into department budget.

  • Works with multiple departments on shared goals

  • Support the financial goals and strategic plans of the hospital.

  • Demonstrates continued knowledge and commitment to professional growth and competence in Care Management

  • Provides input and actively participates in performance improvement activities, staffing analysis, weekend activities and data collection as directed by the department.

  • Maintain a high level of patient/customer service.

  • All unit coverage as indicated.

  • All other duties as assigned.

Qualifications

Required Qualifications: NY State RN License. Master's Degree. Minimum five years acute care experience. Experience in Care Management. Certified RN, CM or CDI. Working knowledge of Inpatient Status, Observation, RAC, Cert, MAC. Understanding of discharge planning including barriers to discharge. Experience with Unit Rounds. Knowledge of transitional care. Interview skills/high level CSR service with demonstrated leadership experience. PRI Certified. Weekday and weekend flexibility, day and evening rotation as needed.

Preferred Qualifications: Other certifications for RN i.e. Quality, MCG, etc. Recent tertiary care experience. Experience in data base systems e.g. excel, word and access and hospital systems. Working knowledge of MCG and/or Interqual criteria. Track, trend and analyze reports with documentation. Managed Care or IPRO experience. Knowledge of regulatory requirements i.e. CMS, OBS, Two Midnight Stay, EMTALA, etc. Previous Leadership experience. Bilingual.

  • Successfully complete a Background Check investigation.
  • Provide a copy of any required New York State license(s)/certificate(s).
