Job Description
JOB FUNCTION:
The WellLife Network Children’s SPOA Behavioral Health Aide is responsible for providing support to the clinical team of the Suffolk County Children’s Single Point of Access, a centralized referral system for children and youth who have been diagnosed with a serious emotional disturbance which interferes with their ability to function in the home and community.
ESSENTIAL ACCOUNTABILITIES:
Obtain and review pertinent documentation for referral completeness
Contact referral source and other key individuals to gather information
Assist in monitoring referral status and follow-up
Knowledge of waitlists to assist in monitoring and outreach
Handle telephone inquiries requesting information about programs and behavioral health services
Assist families and providers navigate multiple children serving systems, service linkage and referral
Participate effectively in multidisciplinary meetings
Facilitate trainings in the community regarding CSPOA and community services
Design and update program material
Timely and accurate data entry/updates into the appropriate database/EHR
Undertake other duties as needed

Qualifications
QUALIFICATIONS:
BACHELOR’S DEGREE IN BEHAVIORAL HEALTH OR RELATED HEALTH/HUMAN SERVICES FIELD AND 1 YEAR EXPERIENCE. WORKING KNOWLEDGE OF CHILDREN’S BEHAVIORAL HEALTH SYSTEM, A PLUS.
ACTIVE NYS DRIVER'S LICENSE FOR 2 YEARS
