Job Description

JOB FUNCTION:

The WellLife Network Children’s SPOA Behavioral Health Aide is responsible for providing support to the clinical team of the Suffolk County Children’s Single Point of Access, a centralized referral system for children and youth who have been diagnosed with a serious emotional disturbance which interferes with their ability to function in the home and community.

ESSENTIAL ACCOUNTABILITIES:

  • Obtain and review pertinent documentation for referral completeness

  • Contact referral source and other key individuals to gather information

  • Assist in monitoring referral status and follow-up

  • Knowledge of waitlists to assist in monitoring and outreach

  • Handle telephone inquiries requesting information about programs and behavioral health services

  • Assist families and providers navigate multiple children serving systems, service linkage and referral

  • Participate effectively in multidisciplinary meetings

  • Facilitate trainings in the community regarding CSPOA and community services

  • Design and update program material

  • Timely and accurate data entry/updates into the appropriate database/EHR

  • Undertake other duties as needed

Qualifications

QUALIFICATIONS:

  • BACHELOR’S DEGREE IN BEHAVIORAL HEALTH OR RELATED HEALTH/HUMAN SERVICES FIELD AND 1 YEAR EXPERIENCE. WORKING KNOWLEDGE OF CHILDREN’S BEHAVIORAL HEALTH SYSTEM, A PLUS.

  • ACTIVE NYS DRIVER'S LICENSE FOR 2 YEARS
