Responsibilities:
Provide patient-centered care as a key member of the healthcare team, using sound nursing judgment and focused assessment.
Collaborate with RNs and healthcare team members to address patient needs beyond LPN scope, skill level, or when a patient’s condition changes.
Follow UPMC policies, LPN Practice Guidelines, and State Practice Act in all nursing duties, including IV therapy.
Develop daily patient care plans with a holistic approach, involving patients as active partners in their care.
Maintain thorough and accurate patient documentation to ensure seamless communication among caregivers.
Deliver patient education to support care transitions and promote overall health.
Foster a caring and compassionate environment, building strong relationships with patients, families, and colleagues.
Actively participate in unit activities and support team members in providing high-quality patient care.
Mentor and support new LPNs and patient care staff, contributing to professional growth and development.
Engage in ongoing learning and participate in initiatives to improve patient safety, quality, and workplace efficiency.
Promote a healthy work-life balance, demonstrate accountability, and maintain a positive, professional attitude.
Identify and report safety hazards while upholding a culture of collaboration, respect, and continuous improvement
Graduate of a State Board-approved Licensed Practical Nursing (LPN) program.
Strong ability to assess and respond to patient needs across all ages and conditions.
Skilled in process improvement and collaboration with healthcare teams and patients/families.
Ability to work in a fast-paced environment, managing multiple priorities and specialized equipment.
Physical stamina for frequent walking, standing, lifting, and patient positioning.
Licensure & Certifications:
Current LPN license in the state of practice or a multistate license under the Nursing Licensure Compact (NLC).
Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification.
Comprehensive Crisis Management (CCM) training within 60 days of hire and annually thereafter.
Act 34 clearance required
