Responsibilities:

  • Provide patient-centered care as a key member of the healthcare team, using sound nursing judgment and focused assessment.

  • Collaborate with RNs and healthcare team members to address patient needs beyond LPN scope, skill level, or when a patient’s condition changes.

  • Follow UPMC policies, LPN Practice Guidelines, and State Practice Act in all nursing duties, including IV therapy.

  • Develop daily patient care plans with a holistic approach, involving patients as active partners in their care.

  • Maintain thorough and accurate patient documentation to ensure seamless communication among caregivers.

  • Deliver patient education to support care transitions and promote overall health.

  • Foster a caring and compassionate environment, building strong relationships with patients, families, and colleagues.

  • Actively participate in unit activities and support team members in providing high-quality patient care.

  • Mentor and support new LPNs and patient care staff, contributing to professional growth and development.

  • Engage in ongoing learning and participate in initiatives to improve patient safety, quality, and workplace efficiency.

  • Promote a healthy work-life balance, demonstrate accountability, and maintain a positive, professional attitude.

  • Identify and report safety hazards while upholding a culture of collaboration, respect, and continuous improvement

  • Graduate of a State Board-approved Licensed Practical Nursing (LPN) program.

  • Strong ability to assess and respond to patient needs across all ages and conditions.

  • Skilled in process improvement and collaboration with healthcare teams and patients/families.

  • Ability to work in a fast-paced environment, managing multiple priorities and specialized equipment.

  • Physical stamina for frequent walking, standing, lifting, and patient positioning.

Licensure & Certifications:

  • Current LPN license in the state of practice or a multistate license under the Nursing Licensure Compact (NLC).

  • Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification.

  • Comprehensive Crisis Management (CCM) training within 60 days of hire and annually thereafter.

  • Act 34 clearance required
