Job Details Responsibilities: Assists with Social Work Services to patients from admission to discharge. Participates in the interdisciplinary team care process. Interacts effectively with external agencies in regard to discharge planning issues. Advocates for the optimal discharge plan for 3-K patients. Leads therapeutic groups for patients. Provides clear and comprehensive documentation.. Communicates effectively and with the interdisciplinary team. Is knowledgeable about the intricacies of local discharge planning options. Maintains productive relationships with patients. Is self-directed and able to work independently as needed. Qualifications: BA/BS in Social Work or related field. Previous casework experience. Experience with the mental health population. Salary Range USD $26.22 - USD $29.45 /Hr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience.
