Description

Provide support to TBI and NHTD staff within the CCFCS Aging Services Division with the implementation of meaningful service provision to clients living in Monroe, Finger Lakes or Southern Tier Regional Counties who have been diagnosed with a disability and/or Traumatic Brain Injury. Effectively supports ongoing and collaborative communication amongst all program staff and is responsible for ensuring a trauma informed care environment.

Essential Duties and Responsibilities

  • Assists staff with client appointments and accessing needed services as identified on client’s care/service plans.

  • Assists staff in providing client-guided meaningful service(s) for participant(s) monthly, as required or when needed, to ensure all service plan goals are addressed to the satisfaction of the clients served, maintaining accurate and verifiable documentation of services rendered.

  • Demonstrates, promote and practice cultural competency towards clients through respect and understanding.

  • Recognizes and embrace diversity through supporting and encouraging the strengths of each individual served.

  • Provides support to clients in completing applications and paperwork for the management of benefits, financial, housing, or other related areas as needed.

  • Provides support to staff with attempts to contact clients who have disengaged from services.

  • Utilizes a team approach, maintain accessibility for clients during the workday.

  • Makes independent decisions and recommendations regarding immediate risk factors presented and provide crisis intervention and follow-up as needed.

  • Serves as client advocate.

  • Participates in staff meetings and other Care Coordination and Aging Services Division activities.

  • Ensures compliance with all applicable local, State, Federal, and Health Home Network regulations, and agency policy.

  • Participates in all mandatory training.

  • Represents CCFCS on appropriate boards and committees as assigned by Care Coordination Director.

  • Assists with the maintaining of participant files and records.

  • Assists Program Supervisor or designee with gathering of required documentation to establish and/or maintain eligibility.

  • Participates in relevant Agency meetings and/or trainings

  • Actively pursues development of professional competencies related to job role performance through reading, supervision, in-service training and attendance at conferences and workshops.

  • Awareness and active support of the Agency's Mission, Vision, Values and Strategic Plan. Including, but not limited to, supporting the Agency's Core Competency initiatives of being Collaborative and Community Focused, Innovative and Entrepreneurial, and Culturally Competent and Diverse

  • Other duties as assigned

Qualifications

Education: High School Diploma/GED and two years of human service experience or Associate Degree in health or human services and one year of human service experience. A combination of education and experience will be considered.

Experience: Bilingual (English/Spanish) is required for some positions. If position required bilingual skills to be used, additional compensation will be offered.
