Conemaugh Nason Medical Center is seeking a full-time Registered Nurse for our Cardiac Rehabilitation Center.The Cardiac Rehab Registered Nurse is responsible for assessing, planning, implementing, and evaluating education and exercise programs for rehabilitation of the cardiac patient – to achieve optimal patient outcomes. Collaborates with Medical Director to establish exercise plan. Provides care to adult patients through geriatric patients.

Essential Functions:

Coordinates the Cardiac Rehab Team’s delivery of personalized, efficient care with respect for patients’ spiritual and cultural differences.

Is a resource for patients and families in meeting established goals and outcomes.

Conducts intake interviews and completes assessment of candidates for the program, considering factors related to safety, effectiveness and cost in planning and delivering patient care.

Documents pertinent data in patient’s record; patient chart for entry into program.

Develops individualized plan of care and exercise prescription involving patient, family, and Medical Director to establish goals to achieve desired outcomes; implement plan of care and evaluates patient response.

Initiates and teaches educational components to patient, family and community.

Discharge planning is completed with involvement of patient, family, and cardiac rehab team.

Assesses learning needs of patients; provides teaching and evaluates patient understanding and outcome.

Collaborates with other departments to achieve outcome.

Assumes responsibility, authority, and accountability for clinical practice.

Makes decision and judgments in preventing, analyzing, and resolving patient care concerns.

Maintains confidentiality regarding patient/employee/hospital information.

Maintains annual competencies.

Attends and participates as a minimum requirement of 50% staff meetings/ annually. Reads and signs off all staff minutes of all staff meetings not attending.

Demonstrates ability to adapt to various specific patient population.

Demonstrates ability to operate and maintain all equipment in accord with instructions, policies, and procedures. Notifies the Cardiac Rehab Manager of any problems with the equipment.

Follows safety procedures and guidelines. Reports unsafe conditions.

Maintains safe handling of hazardous and infectious materials in accordance with Hospital policy.

Enters complete and accurate data into the established database.

Uses all Cardiac Rehab computer system for work related activities. Safeguards all computer information and maintains confidentiality. Proves ability to navigate all computer programs.

Demonstrates clinical competence in basic cardiac physiology/pathology, telemetry, 12 lead EKG interpretation; pacemaker operations (implantable) related to cardiac care.

Job Requirements:

Education:

Graduate of an approved nursing program required, Bachelor's degree preferred.

Minimum Work Experience:

Minimum of 2-years of experience in a Critical Care or Cardiac related field required.

Required Skills:

Ability to apply critical thinking skills, knowledge base, physical tasks and interpersonal communication skills.

Skilled at demonstrating and implementing all core and job specific competencies/behaviors.

Required Licenses:

Current nurse's license in the state of Pennsylvania or a current multistate license (MLS) from compact member states/territories required.

Certifications:

BLS Certification is required.

ACLS Certification or obtained within 90-days.
