Full-Time Clinical Social Worker - Social Work

Reports To: Social Work Manager

Responsibilities :

• Provide individual and family counseling for patients and families

• Conduct psychosocial assessment

• Formulate and implement the intervention plan

• Provide crisis intervention for patients who are in crisis

• Provide intensive case management/care coordination service for patients with complex bio-psychosocial needs

• Provide advocacy and referral service for patients

• Supervise interns and volunteers

• Complete documentations and reports

• Attend case conferences, agency meetings and staff meetings

• Conduct group/workshop for patients and the community

• Participate in community outreach and marketing activities

• Perform other duties as assigned

  Minimum Qualifications :

• Master's Degree in Social Work from an accredited social work school

• New York State LMSW/LCSW License required

• Basic CPR (BCLS) Certification required (Health Center will provide if needed)

• Working experience in social services field preferred

• Familiar with the Asian-American communities

• Good verbal and written English skills

• Bilingual in English/Chinese required

  Compensation and Benefits :

• Salary: $65,000.00 - 68,000.00 per year
