Pediatrics Nurse Practitioner (Part-Time) - Queens

The Charles B. Wang Community Health Center, an award-winning federally qualified community health center is seeking a part-time Pediatrics Nurse Practitioner to join our practice. We are looking for team-oriented individuals interested in delivering high quality primary care in a community health setting. Our collegial outpatient-only practice offers strong ancillary support, including nursing support, referral coordinators, health educators, on-site mental health services, and social workers.

We do not require hospital rounding. Call is limited to telephone only. We also provide opportunities for teaching, research, and community outreach.

Responsibilities :

  • Involve with direct patient physical assessment, education, and health planning

  • Maintain continuity of care by regular patient follow-up

  • Assist in the coordination of services with other disciplines and patient's insurance plans

  • Advocate for patient in arrangement of medically necessary services

  • Communicate with community's resources to ascertain adequate assistance for patients in need

  • Refer patients to specialty services when necessary

  • Consult with collaborating physician and other members of the health care team to establish plan of care

  • Participate in quality assurance process to assure optimal care delivery

  • Provide comprehensive case management services that include emphasis on health promotion and disease prevention

  • Monitor patient outcome in accordance to the practice protocols

  • Collaborate with staff nurses and nursing administrators with assessment and clinical decision in patient care planning

  • Act as a resource person for the nursing staff

  • Able to work 3 days per week including weekends

  • Perform other duties as assigned

    Minimum Qualifications :

  • Graduated from approved Nurse Practitioner Program

  • Licensed as Nurse Practitioner in New York State

  • Basic CPR (BCLS) certification required

  • At least 3 years of clinical experience as a Pediatrics Nurse Practitioner

  • Specialty Certification preferred

  • Bilingual in English/Mandarin-Chinese/English or English/Spanish required
