Finger Lakes Community Health (FLCH) was founded in 1989 with an original mission of serving the region's agricultural workers by linking individuals to area resources and providing essential health services. In 2009 the organization expanded to become a Federally Qualified Health Center (FQHC), serving patients of all incomes, ethnicities and walks of life. Over the last decade, FLCH has grown to provide medical, dental, and behavioral health services to over 28,000 patients at eight health centers. Job Summary :
The Patient Navigator is responsible for navigating health care services alongside patients who are high-risk or have barriers to care to promote better health outcomes. The position facilitates communication between patients, family members, healthcare providers, and community services.
Main Responsibilities :
Patient Risk Identification
Identify high-risk patients based off of clinical department referrals and actions
Refer high-risk patients to the appropriate Finger Lakes Community Health department, outside agency, or community resource
Contact patient prior to appointment based on provider request and identified risk / patient needs
Schedule transportation through outside agency for eligible high-risk patients
Refer uninsured or underinsured patients to Financial Advocacy team for health insurance enrollment
Appointment Services
Guide patient through completion of forms including:
Patient intake forms
Workers compensation forms
Sliding fee application
Assist patient with referral and/or follow-up appointment scheduling
Educate patients on prescription medication discounts
Ensure understanding of appointment information through use of printed educational materials and teach-back method
Educate patients on community and social services resources through verbal discussion and printed education materials
Provide interpretation services for patients for the duration of the patient on-site or telehealth visit
Assist patients with voucher program eligibility requirements and appointment scheduling
Teledental Program
Serve as bridge between the patient, Finger Lakes Community Health, and partnered teledental program including:
Patient follow-up
Appointment coordination
Addressing barriers to care
Documentation
Submit telephone encounters regarding patient vouchers for processing
Record progress notes and patient education information in the electronic health record system
Address telephone encounters, actions, and referrals in the electronic health record system in accordance with Finger Lakes Community Health policies and procedures
Education and Qualifications:
High School or Equivalency Diploma
Minimum of 2 years of experience in a similar role
Must be able to fluently speak in both English and Spanish languages
Experience with interpretation or interpretation certification preferred
Possesses New York State driver’s license
Current New York State automobile insurance in compliance with FLCH underwriting agreement
Physical Requirements:
Must be able to hear and communicate with clients and staff via phone, web media, and in person.
Must be able to lift up to 30 lbs
Must be able to sit for extended periods of time
Must be able to kneel, crawl, or bend into awkward spaces
OSHA Level II
