Penn Medicine Princeton Health is hiring for a Director of Patient Safety. We are looking for someone recent/direct experience working with safety, risk and/or regulatory management. Experience in Performance Improvement/Quality will also be considered.

The Director, Patient Safety reports to the Vice President and Chief Medical Officer (CMO) and works in partnership with the Director, Clinical Effectiveness and Quality Improvement and PMPH Patient Safety Officer and is responsible for developing, leading, and building an organization-wide world-class patient safety program placing the entity at the forefront of patient safety. Additional responsibilities include regulatory compliance, coordination of related data collection, analysis and reporting related to patient safety; collaboration with professionals both at the entity and system level to identify and implement patient safety improvement initiatives to achieve departmental and organizational strategic goals. The Director of Patient Safety relies on extensive experience and judgment to plan and accomplish goals. The Director of Patient Safety will work directly with the Director of Quality, the Senior Leadership team and Medical Staff leadership to lead the organization to outstanding performance on key metrics in quality and patient safety.

Key Roles and Responsibilities:

  • Responsible for risk and regulatory management.

  • Support the operations of the PMPH Process Improvement department

  • Responsible for the developing, building, and leading an organization-wide world-class patient safety program placing the entity at the forefront of patient safety

  • Partners with Department and Executive Leaders

  • Serve as entity lead for HRO efforts related to patient safety (e.g. Culture of Safety survey)

  • Responsible for leading and championing continuous process improvement via the application of patient safety science at the entity

  • Support the operations of the PMPH Process Improvement department

  • Partners with Department and Executive Leaders

  • Responsible for leading and championing continuous process improvement via the application of patient safety science at the entity

  • Supports learning, development, teaching/training, and maintenance of processes necessary to integrate clinical performance activities at PMPH.

  • Lead and collaborate in the implementation of the hospital's patient safety plan, to eliminate medical errors through timely reporting, early identification of systems defects and implementation of solutions that promote patient safety.

  • Key teammate with the CMO, Director of Quality, and the entity Patient Safety Officers in engaging executive, entity and clinical leadership to foster a culture of safety and improve patient outcomes.

  • Serve as entity lead for HRO efforts related to patient safety (e.g. Culture of Safety survey)

  • Responsible for leading and championing continuous process improvement via the application of patient safety science at the entity

  • Supports learning, development, teaching/training, and maintenance of processes necessary to integrate clinical performance activities at PMPH.

  • Lead and collaborate in the implementation of the hospital's patient safety plan, to eliminate medical errors through timely reporting, early identification of systems defects and implementation of solutions that promote patient safety.

  • Serves as entity administrator of Midas, Penn Medicine’s event reporting system, by routing event reports and onboarding of PMPH staff and providers, as needed.

  • Reviews reports for trends that may require action, including reports that monitor manager and director event management and follow up. Serves as PMPH “watchdog”, ensuring appropriate assignment of harm score and event management routing.

  • Leads serious event management processes and their outcomes, including root cause analyses (RCAs), in conjunction with the PMPH Director of Patient Safety, Quality and Risk and PMPH Patient Safety Officer to ensure thorough and efficient event evaluation and institutional response, including necessary internal and external reporting (New Jersey Department of Health and The Joint Commission).

  • Collaborate with the Patient Safety Committee in its oversight of the PMPH patient safety plan, including, but not limited to, coordinating the monitoring of compliance to standards supporting the National Patient Safety Goals, Sentinel Events, Leapfrog Hospital Survey, Culture of Safety, FMEA and patient safety related projects.

  • In conjunction with Health Information Management and the Performance Improvement team ensure accurate coding and reporting of Patient Safety Indicators, and identify occurrences where clinical or performance improvement follow-up and action are warranted.

  • Coordinate all local efforts that support the implementation of the AHRQ Patient Safety Culture Survey and develop, maintain, and refine standardized reports to support and improve Patient Safety culture and measurable outcomes, and present these reports and other information, as needed, to relevant departments and/or committees

  • Work collaboratively to embed excellence in patient safety and quality improvement.

  • Contribute to generating and sharing new knowledge in regional, state and national patient safety venues in conjunction with UPHS (i.e. University Health System Consortium (UHC), CMS, IHI, NPSF, etc.)

  • Prepare materials, including agenda, PowerPoints, case reviews, etc. when requested to support CMO and other senior leaders and/or participate in senior meetings such as BOT Quality, System-Wide Leadership, CEQI, and Executive Safety Committee.

  • Serves as entity administrator of Origami, Penn Medicine’s potential compensable event database. Gathers necessary information and acts as a liaison with the liability insurance carrier regarding all potential and verified claims.

  • Coordinates the release of records in response to subpoenas, insurance, or attorney requests.

  • Works with appointed hospital legal counsel regarding the investigation of forthcoming proceedings, and acts as a liaison with hospital employees concerning their interviews/depositions, when appropriate, to prepare defense claims against the hospital.

  • Coordinates the collection of documents, and other information, and prepares written reports in order to ensure timely and complete responses to hospital-appointed attorneys related to requested information to prepare for pending litigation.

  • Advises the PMPH Director of Quality of any complex issues or problems involving patient safety and recommends corrective actions.

  • Leads Safety Huddles and other safety meetings, whether standing or ad-hoc, as reasonably requested.

Key Qualifications:

  • Registered Nurse - NJ (Required)

  • Cert Professional in Healthcare Risk Management

  • Certified Professional in Patient Safety

  • Bachelor of Arts or Science (Required)

  • Education Specialization: Equivalent Experience:

  • Master of Arts or Science

  • 3+ years of Management experience
