We are currently seeking a dedicated Per Diem Physical Therapist (PT) to join our team. This role offers flexibility for PTs looking to work on a per diem basis while making a meaningful impact on patients' rehabilitation and recovery.

Job Description

As a Per Diem Physical Therapist, you will assess and treat patients in their homes, creating and implementing personalized physical therapy programs to help patients regain mobility, strength, and independence. You will collaborate with a multidisciplinary team to ensure optimal patient outcomes and improve quality of life through effective rehabilitation techniques.

Key Responsibilities

• Conduct comprehensive assessments of patients' physical conditions, mobility, and functional abilities.

• Develop and implement individualized treatment plans tailored to the patient's specific needs and goals.

• Provide physical therapy interventions including exercises, manual therapy, mobility training, and pain management techniques.

• Monitor and document patient progress, adjusting treatment plans as needed to optimize results.

• Educate patients and families on home exercises, safety strategies, and techniques for improving mobility and strength.

• Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated and holistic care.

• Ensure compliance with safety, regulatory, and infection control guidelines.

• Maintain accurate and up-to-date documentation of patient care and progress.

   

Qualifications

• Current Physical Therapist (PT) license in PA.

• Minimum 3 years of experience as a Physical Therapist, preferably in home health care or rehabilitation settings.

• Strong clinical assessment and problem-solving skills.

• Ability to work independently and manage a flexible, per diem schedule.

• Excellent communication and interpersonal skills.

• Compassionate and patient-focused approach to care.

• Valid driver's license and reliable transportation.

Preferred Qualifications

• Experience in home health care, geriatrics, or working with post-surgical or neurological patients.

• BLS/CPR certification.

• Manual therapy or other specialized certifications.

• Bilingual (preferred but not required).
