Job Description/Essential Functions:

Horizon House, Inc. is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse. Horizon House is Pennsylvania's oldest psychiatric rehabilitation agency. Since 1952, we have been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities, and homelessness to stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout South and North Eastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice. Currently, we are seeking a Full-time Behavioral Health Clinician to work in our Behavioral Health Services Division. Purpose: * To provide support, education, counseling and guidance to clients in the recovery process. * To develop and reinforce mechanisms and resources to facilitate a substance free life style. * To ensure a safe, secure and pleasant environment, in compliance with Agency policies and procedures and applicable regulations. * To implement the overall mission, values and beliefs of Horizon House in all aspects of the job. * To monitor and track progress of assigned caseload. * To provide individual and group outpatient counseling services for an assigned caseload of clients. Supervision Received: * Receives direct supervision from the Program Manager. Supervision Exercised: * N/A Other Duties Include: *Provide individual, group and/or family therapy according to each individual's recovery plan. * Complete and maintain all chart documentation in compliance with insurance, licensing, and Medicaid Certification standards including progress notes, recovery plans, and clinical reviews within established timeframes. * Ensure each client has input into developing an up-to date recovery plan; modify plans to adapt to special needs and/or service delivery gaps. * Establish an on-going dialogue with other members of the rehabilitation/treatment team, including care managers, vocational/educational staff, consultants, members of the medical profession and family members, as needed, to provide continuous supports; schedule planning and linking conferences as needed

Job Qualifications:

Qualifications: * Masters degree in Psychology, Rehabilitation Counseling, Social Work, Nursing, or related field from an accredited college or university (or a registered nurse with a certificate in mental health nursing from the American Nurses Association) .Certified Alcohol and Drug Counselor (CADC) preferred - if not fully certified, must complete full certification within 5 years of hire date. Must submit annual personnel file updates documenting progress toward full certification. * Knowledge of a variety of Mental Health Evidence Based Interventions. * Knowledge of multiple support systems for mental health and substance abuse clients. * Good communication skills. * Ability to work independently and as part of a team. * Excellent documentation skills. * Must possess a valid driver's license with an acceptable driving record. Scope: * Frequent talking and listening. * Requires the ability to lift, carry, fold, kneel, reach, stack, stoop and bend which may include direct intervention with residents. * Must be able to assist in the evacuation of clients during emergency situations
