Niagara County. Adult Health Homes Care Manager. Intandem. FT. 1st Shift. Benefits. $21/hour.

POSITION SUMMARY: Works with people with qualifying Physical/Mental Health/Substance Abuse/Developmental Disability diagnoses and their natural supports to link/monitor/refer to services that meet their individual needs and personal goals. Also provides ongoing advocacy.

QUALIFICATIONS: Minimum Education, Training & Experience: - Bachelors Degree in Human Service related field Or - Credentialed Alcoholism and Substance Abuse counselor (CASAC) And - Two years of experience providing Care Management services (Case Management) to individuals with qualifying factors; i.e. physical/MH/ Substance diagnosis

Preferred Education, Training & Experience: The above plus two (2) years experience developing Care Plans

ESSENTIAL SKILLS AND ABILITIES: * Valid New York State drivers license and dependable transportation that meets the Agencys driving policy * Ability to work flexible hours and travel throughout Niagara County * Ability to work effectively and independently including address multiple tasks efficiently * Possesses strong organizational, analytical and creative thinking skills * Ability to effectively communicate verbally and in writing including developing rapport with the people we serve * Ability to represent self and agency in a professional manner * Possesses strong knowledge of community resources * Willingness and ability to participate in an on-call rotation

SALARY: $21 per hour starting rate
