We treat each patient with respect and dignity and provide an outcome-driven and fully integrated continuum of care that addresses the diverse needs of our patients.The Lennard Clinic has been servicing these counties for more than 35 years and is looking for qualified candidates interested in the behavioral health field to provide individual and group counseling services in an outpatient Medication Assisted Treatment (MAT) facility.Essential Duties and Responsibilities-Responsible for scheduling clients for appointments and ensure clients keep sessions, and if missed to document.-Responsible for writing and submitting progress notes daily.-Assess the counseling needs of a patient.-Assess clients using the ASI or other standardized clinical interview tool, and diagnosing clients for substance use disorder using the DSM-5.-Determine the appropriate level of care according to ASAM Criteria.-Obtain previous records that are relevant to the current treatment episode.-Unless clinically contraindicated, collaborate with the client to develop a written treatment plan that includes goals and measurable objectives, and is client centered, recovery oriented.-Provide the substance abuse counseling services specified in the patient treatment plan.-Ensure discharge planning in accordance to ASAM Criteria to determine the need for continued services, transfer, or discharge.-Review and revise client treatment as necessary.-Case management as necessary, inclusive of referral sources.-Participate in multidisciplinary discussions.-Document all counseling and education services, assessments, reassessments, referrals and follow-up in the patients’ clinical record, and provide appropriate signatures and dates.-Serve as liaison with medical department-Provide clients with orientation regarding objectives of the program, phase system requirements, rules and regulations, sanctions, hours of service, clients’ rights and additional information to help clients adjust to the program.-Assure each client is assessed one of the following Phases and provide counseling to that patient in accordance with the following schedule:Phase I - At least one counseling session per week.Phase II - At least one counseling session every two weeks.Phase III - At least one counseling session every month.Phase IV - At least one counseling session every three months-Submit monthly and quarterly statistical reports to supervisor.-Perform quarterly treatment plan reviews, monthly case reviews when appropriate and participate in case conferences in accordance with established policies and procedures.-Submit Progress Notes on a weekly basis to supervisor for review and approval.-Ensure that all clients receive a minimum of 12 random urine screens per year in Phase I and II and at least eight (8) urine screens per year in Phase III and IV, except that any client receiving take home bottles shall have random monthly screenings.-Perform additional duties as assigned.Qualifications, Experience, Education and LicensureCADC RequiredBachelor’s degree in Psychology, Social Work, Counseling or related field preferred but not required.One-Two years of substance abuse counselingHave technical skills with cloud-based software (Google Suite (Email, Docs, Drive, Sheets, etc), Microsoft Office, One Drive, SMART (preferred))CPR certified preferredJob Type: Full TimeSalary:$50,000-$55,000Performance Reviews are conducted on a semiannual basis Benefits-Dental insurance-Employee assistance program-Health insurance-Life insurance-Paid time off-403B-Vision insuranceShift:7 hoursDay shiftMonday-FridayThe Lennard Clinic is committed to inclusive hiring and dedicated to diversity in our work and workplace culture.
