Project FIND Lead Social Worker in New York, New York

Company Description:Project FIND’s mission is to provide low- and moderate-income and homeless seniors with the services and support they need to enrich their lives and live independently. Today Project FIND operates three supportive housing residences that are home to about 600 people and four senior centers with over 3,000 members. Our members and residents range from the healthy and active to the frail and homebound to the homeless. For all these individuals, Project FIND is a critical resource, providing housing, meals, and programs that help individuals navigate the challenges of aging by encouraging community engagement and healthy living.Project FIND’s Older Adult Centers are dedicated site that provides a variety of meals, activities, and services to people over the age of 60. Job Summary:The Lead Social Worker will work with older adults living at the Park 79 location. The program provides case management services, information, referrals, counseling, housing assistance, group work and crisis intervention for clients and their families. The Lead Social Worker is responsible for conducting detailed assessments to determine client needs and eligibility for services and engaging with clients to develop a service plan. General Responsibilities:-Provide conflict resolution and engage in tenant concerns in matters requiring enhanced intervention.-Responsible for any monthly, quarterly, and any other type of reporting requested of the program by its funder and/or organization. -Oversee staff scheduling and ensure staff maintain a presence at the residence during business hours. -Interface with the Property Management team to reconcile outstanding items, and to resolve ongoing concerns or issues related to residence, including both tenant and facility concerns and issues.-Conduct intake and assessment screening, assessing mental and physical health needs and supports, and financial eligibility for benefits, entitlements, conduct home and office and community visits to address unmet needs. -Implement constructive short and long-term interventions through counseling education about counseling supports, long-term planning, and assistance with benefits. -Develop and create internal and external community building events and opportunities to facilitate activity and socialization that can help support clients’ ongoing wellness and quality of life.-Lead one or more support groups, providing emotional support and psychoeducation about various issues related to self-care, health and wellness, legal planning, and aging in place.-Provide timely documentation and maintenance of client files and progress notes.-Perform other related tasks as requested or assigned.Qualifications include:-Master’s degree in Social Work required. -LMSW or LCSW strongly preferred with at least 2 years relevant experience in social services; knowledge of benefits and entitlements a plus. -Experience working with older adult populations and families preferred. -Bilingual language Chinese or Spanish fluency preferred.-Must have strong computer skills including Microsoft Office. -Ability to provide accurate and timely documentation required. -Time management skills to coordinate multiple responsibilities and services. -Ability to work independently and as part of a team in an office environment. -Maintain client and agency confidentiality. -Excellent interpersonal, public speaking, and written communication skills. Must be able to communicate effectively and appropriately with other team members, community agencies, and clients with diverse opinions, values, characteristics, and cultures. Schedule:Monday-Friday from 9am-5pm (35 hours per week). Please note this position requires the availability and flexibility to occasionally work some weekends and holidays.Project FIND participates in E-Verify, which confirms employment authorization through an electronic database maintained by the Social Security Administration and Department of Homeland Security. Minimum Salary: 32.96 Maximum Salary: 35.71 Salary Unit: Monthly
