Position: Registered Nurse

Requirements:

• Minimum of two years of home health experience as a Registered Nurse

• Valid and current nursing license in New York

• Demonstrated expertise in delivering quality care in a home health setting

• Strong communication and interpersonal skills

• Ability to assess, plan, and implement effective patient care plans

• Knowledge of regulatory standards and compliance in home health care

Responsibilities:

• Conduct comprehensive patient assessments and develop individualized care plans

• Administer non medical nursing care in accordance with care plans

• Collaborate with the interdisciplinary team to ensure holistic and personalized care

• Provide patient and caregiver education on health management and preventive care

• Monitor and evaluate patient progress, adjusting care plans as needed

• Ensure compliance with all relevant regulations and maintain accurate patient records

• Effectively communicate with care team members
