Values into Action supports people with disabilities one person at a time, on their own terms, and exclusively in their own homes and communities.

 

This position supports people with disabilities and their families in directing their own services. The Support Broker provides information; guidance and technical assistance to the individuals who choose to self-direct their services so that s/he can access, organize and utilize opportunities, resources and services necessary to live a self-determined life.

 

This is part-time (that will transition to full-time), hybrid / remote *position based in the Southeastern region of PA. Candidates must live in and be willing to travel within Bucks, Delaware, Montgomery, Chester, and Philadelphia counties and be familiar with the communities served.*

 

The role of the Support Broker is uniquely defined by the individual utilizing the brokering service, but often includes the following components:

  • Promoting best practices in employee management, supporting Managing Employer and Common Law Employer responsibilities per parameters of Office of Development Programs and Fiscal Management entities.
  • Promoting civil and personal rights-Working as an effective member of an interdisciplinary team to include interaction with community entities, supports coordination offices, and county regulatory bodies.
  • Partnering to ensure the best possible planning and assisting individuals and families with life course planning goals
  • Ensuring the best possible use of public funding, and advising individuals and families regarding all potential services offered through their P/FDS, Community Living and Consolidated waivers.

     

A Support Broker will demonstrate the following abilities:

  • Function within a participatory management structure;
  • Self-direct work; to include independent research and community networking on behalf of individuals and families served.
  • Work effectively as a member of a team;
  • Communicate effectively both orally and in writing; good presentation skills within interdisciplinary team settings.
  • Working knowledge and application of basic employment law, principles of self-determination, person centered thinking and planning.

     

Qualifications

Ideal Candidate:

  • Will be a current Certified Supports Broker (strongly preferred) and have at least two years of experience in a supervisory role, HR management, or case management.
  • A valid drivers license with a safe driving record and a fully registered, inspected and insured vehicle to use while working OR a reliable transportation alternative.
  • Experience working within 6400, ICF/ID, or familiarity with regulatory requirements set forth by Office of Developmental Program
  • Be fully Covid-19 vaccinated prior to employment

Qualifications
