Job Description

Responsible for coordinating and facilitating the discharge process for transplant recipients, acting as a liaison to inpatient care team, anticipating barriers (i.e., including prior authorizations, lab orders, and placement to subacute rehab facilities) and expediting transitions of care/discharge planning.

Job Responsibility

  • Takes initial patient histories and performs physical examinations.

  • Initiates, documents, and communicates the plan of care and follows up to ensure complete screening and preparation of patients.

  • Performs daily patient rounds.

  • Writes orders for medications, laboratory work, and diagnostic tests.

  • Interprets laboratory and test results.

  • Confers with attending physicians, residents, nursing staff, and/or other care providers to ensure optimum quality of patient care.

  • Upon discharge, notes discharge summaries, prescriptions, and any referrals on patient charts.

  • Informs patients of the necessary post-discharge care instructions.

  • Demonstrates knowledge and ability to coordinate the care of the transplant recipient to achieve optimal physical, social, and emotional rehabilitation

  • Identifies and responds to recipient/family/legal guardian's educational, psychosocial, and economic needs in discharge process and post-transplant phase

  • Ensures patients and caregivers receive adequate education throughout the discharge process (i.e., medication, post-discharge follow-up and care, etc.)

  • Makes referrals to social work and other members of the care team as identified and expedites consults with the physician regarding physical therapy, nutrition, speech therapy, respiratory therapy and other ancillary services as needed

  • Coordinates and monitors prior authorizations, lab orders, testing, and discharge consults in a safe and timely manner as directed by advanced care practitioner and/or physician

  • Identifies and assists in removing barriers to discharge planning, patient care and resolves issues with appropriate departments and staff as directed by advanced care practitioner and/or physician

  • Initiates discharge planning by assessing the patient's needs and documenting the assessment as directed by advanced care practitioner and/or physician and in partnership with case management and social work

  • Ensures patient and/or family are given information regarding their choices regarding transfer to another level of care according to regulatory standards in collaboration with the clinical care team

  • Ensures continuing care services including transportation, durable medical equipment, etc. are appropriately arranged for and financially approved

  • Documents ongoing process of discharge planning including discharge assessment, plan and on-going evaluation and up-dates. Provides summary note at time of discharge synthesizing the discharge plan and follow-up care needs in collaboration with the clinical care team.

  • Manages discharge coordinator for complex Transplant and Acute Rehab cases and serve as a strong and knowledgeable liaison to physicians, nurses, social workers and other healthcare team members

  • At the direction of the physician and/or advanced care practitioner, assists in ensuring optimal length of stay and utilization

  • With the partnership of the physician and advanced care practitioner, assists in the prevention of hospital readmission, promote and anticipate follow up appointments and consultations

  • Discusses with attending physician and/or advanced care practitioner the appropriateness of resource utilization, consultation and treatment plan

  • Discusses estimated length of stay, treatment and discharge plan with the attending physician/advanced care practitioner and along members of the care team, as indicated

  • Coordinates and facilitates transitional planning needs through the acute care continuum

  • Performs related duties as required.

Job Qualification

  • Master's Degree in a Nurse Practitioner Program and National Board Certification in specialty area required.

  • Current license to practice as a Nurse Practitioner in New York State, and current license to practice as a Registered Professional Nurse in New York State, required.

  • Has a collaborative practice agreement with designated protocols filed with NYS Department of Education OR collaborative relationship attestation as per the Nurse Practitioner Modernization Act 2015.

  • Current BLS certification, required.

  • Bachelor's Degree, required. Graduate of an accredited (Accreditation Review Committee for Physician Assistant Programs ARC-PA) program, required.

  • Current license to practice as a Physician Assistant (PA) by the New York State Department of Education, required.

  • Current NCCPA Certification from the National Commission on the Certification of Physician Assistants (NCCPA), required.

  • Current BLS certification, required.

The salary range and/or hourly rate listed is a good faith determination of potential base compensation that may be offered to a successful applicant for this position at the time of this job advertisement and may be modified in the future.When determining a team member's base salary and/or rate, several factors may be considered as applicable (e.g., location, specialty, service line, years of relevant experience, education, credentials, negotiated contracts, budget and internal equity).

The salary range for this position is $100,000-$180,000/year
