Urban Resource Institute Case Manager - Bi-lingual in Jamaica, New York

The Urban Resource Institute (URI), a not-for-profit 501(c) (3) organization established in 1980, operates shelter, legal and job training programs for domestic violence victims, residential and vocational programs for individuals with developmental disabilities. With a $70-80 million annual budget with over 600 staff by the end of the year, URI promotes safety, justice, health, and independence for vulnerable individuals and families residing in New York City.The Case Manager is responsible for direct service provision to residents. Direct services include individual counseling, group counseling, advocacy services, referrals and follow-up, home management and budget counseling. Individual must demonstrate sensitivity to issues related to women and domestic violence. In addition, the Case Manager will make all necessary linkages to community agencies to provide proper placement of residents in areas of permanent housing, referral to engage in after care services prior to leaving the agency, vocational and educational services, and to assist families in securing public assistance, Medicaid or other benefits.POSITION REQUIREMENTS, EDUCATION & ESSENTIAL SKILLS•Conduct intake interviews, as assigned, to assess and evaluate residents for admission.•Engage residents in initial and ongoing treatment processes, through individual andgroup counseling, under clinical supervision of the Superivor of Clinical Service orProgram Director.•Develop and maintain housing referral resources, assist residents in the process oflocating and securing permanent housing, in conjunction with Housing Specialist.•Assists all residents in the process of referral and placement in community basedresources including after care services•Assess residents’ vocational and educational needs, aptitudes and interests throughinterviews and testing. Develop and maintain placement and follow-up as appropriateafter assessment and counseling.•Develop and maintain liaison activities with H.R.A. Public Assistance and MedicaidAdministrators and Workers, to assist eligible residents in securing Public Assistance andMedicaid by acting as a personal advocate through telephone or personal visit/contact.•Participate in training seminars conducted to enhance and develop clinical skills with theclient population.•Attend weekly individual case supervision, group supervision, monthly case conferences,and staff meetings.2 | P a g e•Function independently in time of facility coverage, including, responsibility forappropriate action in crisis situations per agency procedures.•Participate in training seminars conducted to enhance and develop one’s skills.•Regularly assess the safety and well-being of clients•Make appointments for residents’ physical exams.•Develop with the resident, a contract of mutually agreed goals and objectives to beaccomplished within an agreed upon timetable.•Respond to Hotline calls as appropriate•Facilitate psycho educational groups.•Coordinate on-site services such as for child care, child therapy and recreation.•Identify concrete needs and provide on an as needed basis.•Perform other duties as assigned by the Social Work Supervisor.REQUIRED KNOWLEDGE, SKILL AND EXPERIENCE:•High school diploma plus two (2) years of relevant training and experience, including experience working with victims of domestic violence; or Bachelor's degree in health or human services.•Two (2) years of crisis counseling experience preferred in a residential shelter for victims of domestic violence, and experience and demonstrated skills in group counseling.•In addition, must have training and experience in assessing the vocational andeducational needs and aptitudes of clients, thorough familiarity and success withengaging clients experience assisting eligible clients through the process of obtainingPublic Assistance, Medicaid and other benefits.•Must be knowledgeable and sensitive to domestic violence issues•Demonstrated sensitivity and experience with and/or knowledge of domestic violencewithin and against disenfranchised communities including individuals whom identify aslesbian, gay, bisexual, transgender, queer, and/or HIV-affected, the elderly andindividuals with mental health issues.•Must possess a demonstrated ability in delivering client services including:communication and listening skills, understanding of human interaction and personalitydevelopment, the ability to write and record case documentation, and demonstratedskills in group counseling.•Employee must be computer-literate and must have knowledge of Microsoft OfficeApplications.•Must have ability to be flexible of schedule as needed•Excellent written and verbal skills are required

Minimum Salary: 34,000.00 Maximum Salary: 40,000.00 Salary Unit: Yearly
