POSITION SUMMARY:

This Nursing position is within our Mobile Medication Monitoring (MMM) program which is a community based program. The MMM nurse is responsible for providing in-home mobile medication monitoring services to clients in need of skills training, symptom management, and resource coordination. The MMM nurse is responsible for providing mental and physical healthcare education and coordination of medication and related healthcare services; developing and implementing recovery-oriented medication management plans that are comprehensive, accurate and clearly tailored to each client's individual needs; and, conducting ongoing assessments to ensure that goal plans remain relevant, that progress, or the lack thereof, is closely monitored and that modifications can be made as deemed appropriate. The MMM nurse offers clinical consultation and teaching to clients, families and co-workers to ensure that up-to-date information is maintained regarding medical issues. The MMM nurse also participates in the on-call nursing system as scheduled.

MINIMUM QUALIFICATIONS:

  1. Licensed Practical Nurse with NAPNES medication certification; experience working with individuals with mental illness and/or developmental disabilities preferred.
  2. Strong clinical skills with working knowledge of mental health system, psychiatric rehabilitation and recovery principles.
  3. Exceptional communication skills, both written and verbal, organizational skills, knowledge of practical self-care skills and ability to work with minimal supervision.
  4. Ability to effectively work with persons with mental illness and/or developmental disabilities and implement medication monitoring plans for their program participation.
  5. Act 33/34/31 and FBI Clearances
  6. Valid driver's license with access to a vehicle during scheduled work hours.
