Job Description

MAIN FUNCTION:

The Nurse Navigator is responsible for managing a patient’s successful transition from hospital to home, throughout the clinic experience, and ongoing care providing disease management, care coordination and patient triage. The Nurse works collaboratively with interdisciplinary staff internal and external to the organization, participates in quality improvement and evaluation processes related to the management of the Transitional HeartCARE Clinic.

1.

Identifies the appropriate patients for Transitional HeartCare Clinic.

**

2.

Assesses and cares for patients under the direction of the Provider.

**

3.

Provides patient assessment in an ongoing and efficient manner focusing on the patients physical and cognitive needs.

**

4.

Performs accurate triage and assessment of patient problems, prioritizes care and recognizes changes in patient status. Keeps provider informed of changes in plan of care.

5.

Implements an established plan of care in a knowledgeable, skillful and consistent manner.

**

6.

Coordinates deployment of home-based services as appropriate (home health, DME, transportation) per patient needs and resources available.

**

7.

Acts as a resource to the team regarding specialty medications, insurance coverage, and available programs to reduce medication costs.

**

8.

Act as a point of contact for patients enrolled in the Transitional HeartCARE Clinic, performs accurate transitional phone calls post hospital discharge.

**

9.

Demonstrates awareness, sensitivity and knowledge of patient/family learning needs and implements appropriate measures to meet their rights and safety.

**

10.

Documents accurate, concise information into the patients EMR, records weekly Weights and Blood Pressures based on patient need.

Schedules follow up visits, consultations, referred testing per provider orders.

**

12.

Reviews and forwards appropriate information requested by consult providers.

**

13.

Is responsible for contacting the patient regarding lab/test results, phoned in issues after being reviewed by provider and documented in the EMR.

**

14.

Maintains a system to ensure receiving copies of consultation reports and test results in a timely manner with a follow up system for tracking missing information.

**

15.

Demonstrates and functions with awareness of safety issues as identified by the institution towards patients, co-workers and self.

**

16.

Performs efficiently and appropriately in emergency situations, is current with CPR certification.

**

17.

Is responsible for attending all mandatory educational programs as required by position.

**

18.

Demonstrates awareness of basic legal issues in all aspects of patient care, office functions and strives to minimize risks.

**

19.

Is competent to perform/assist provider with patient diagnostic testing such as EKG’s, Event Monitor placement and MCOT placement.

**

20.

Performs all aspects of universal precautions when in patient care and maintains current Infection Control Certification as required by New York State.

21.

Employee understands and demonstrates the importance of satisfying the needs of the customer/patient by interacting with him/her in a friendly and caring way, being attentive to the customer’s needs, both psychologically and physically, and by taking the initiative to maintain communication with the customer to provide a secure and pleasant experience with the Medical Center.

EDUCATION: Graduate of an approved registered nursing program with a current license as a registered nurse.

EXPERIENCE: Minimum of 5 years working as a registered nurse.

Preferred: Experience working in a Cardiology Clinic/Unit
