Job Overview:
As a Licensed Social Worker, you will provide frontline services to patients with conditions spanning the entire health care continuum.
The goal of this acute care, nonclinical position is to discharge patients home or to another safe environment.
As an essential member of the interdisciplinary team, you will work and consult with doctors, nurses, and allied health professionals to sensitize other healthcare providers to the social and emotional aspects of a patient's illness.
Utilizing your case management skills, you will help patients and their families address and resolve the social, financial and psychological problems related to their health condition.
Qualifications:
Required:
Master's degree in Social Work (M.S.W.) from an accredited School of Social Work
Good communication and writing skills
Ability to work in a multidisciplinary team
Preferred:
Bilingual in English and Spanish
Hospital work experience
Certifications and Licenses Required:
- New Jersey State Social Work Licensure (L.S.W. or L.C.S.W.)
Scheduling Requirements:
Shift working, Monday-Friday 8:30-4:30pm
Rotate Saturdays and holidays

Essential Functions:
Identifies, screens and assesses cases from the patient population
Provides counseling to patients and families re: illness, hospitalization and discharge planning
Formulates and implements an appropriate, timely and safe discharge plan for those patients on caseload
Provides community resources, with follow-up, as needed
Maintains monthly statistics, enters appropriate and timely documentation into the patient s medical record re: the Social Work interventions provided
Provides accurate, clear and timely documentation of services provided within the patient s medical record
Participates in interdisciplinary team and maintains collaborative working relationship with all team members
Participates in educational in-services and departmental staff meetings; provides feedback to the Director re: ongoing needs for staff and patients
Attends educational seminars (within the hospital and larger community) and reports significant information, resources to the entire department
Provides field instruction for graduate or undergraduate social work students, if available
Coordinates social work services within the palliative care team and facilitates and/or participates in patient/family meetings for the purposes of goal setting and care planning
Coordinates educational and outreach services relevant to the social work-palliative care collaborative
Provides liaison function between palliative care team and hospital quality improvement, discharge planning and social work colleagues on assigned palliative care cases, with oversight from the Director of Case Management Services
Provides liaison function between the palliative care team and community hospices, home care agencies, long term care facilities and other community agencies as needed on assigned palliative care cases
Documents all social work interventions for palliative care cases in Case Management notes in electronic medical record
