Geisinger Senior, Case Management Assistant - Full Time in Danville, Pennsylvania
Job Summary
Provides seamless appointment scheduling services to patients and referring physician offices by matching physician-specific scheduling guidelines with patient appointment preferences using technology. Facilitates positive multi-channel communications among patients, physicians, clinical staff, outreach sites, and internal departments to serve patient and referring physician needs.
Job Duties
Schedules patient appointments, providing proper prep instructions for ancillary and diagnostic proceduresand tests using physician-specific and department guidelines.
Coordinates and completes accurate basic registration, demographic and insurance information to ensure timely and accurate payment for services while scheduling appointments.
Assures that all appointment requests are satisfied within the time frame specified by the referring provider.
Processes requests for referrals and obtains the referrals for physician offices as required.
Answers patient and referring physician questions pertaining to appointment status.
Processes multi-channel messages related to patient and/or physician requests regarding: appointments, referrals, prescriptions and complaints.
Advises of unusual situations, scheduling system technology or patient-related concerns.
Facilitates and assures physician to physician or department contact.
Coordinates Home Care, Durable Medical Equipment and Supplies, Oxygen, and Transportation.
Identifies gaps in care and social determinants and provides resource education to close gaps.

Position Details
Travel: Although this is WFH, there is an expectation of coming on site for trainings, in person meetings etc., and if productivity is down you will be asked to work on site.
Hours: 8am-430pm with a rotating shift of 830am - 500pm
Required Experience: 1 year of cu stomer Service
Required Education: High School Diploma or Equivalent (GED)
