Shift Schedule : Monday - Friday 8:00am – 4:00pm

Salary : Salary pay range is min. $20.06 to a max. $26.12 per hr. based on education & experience

    Essential Job Functions Overview

    Responsible for implementation and management of Single Point of Entry/Accountability (SPOE/A) program for housing, Non-Medicaid Case Management, and Assertive Community Treatment (ACT) services for Ontario and Seneca counties. SPOA’s goal is to create a system that promotes available services which are recovery-oriented and person-centered to fit the needs of the individuals referred. This includes providing Community Awareness of the available services as well. Individual should strive to create a healing environment that respects the perspectives and experiences of the individuals, families, staff and communities we serve by practicing safe, respectful communication as well as respecting individuals’ boundaries and differences.

    Essential Job Functions

  • Work cooperatively and collaboratively with all referring agents and referred individuals

  • Receive and record all incoming housing, and Non-Medicaid Case Management and Assertive Community Treatment (ACT) services referrals

  • Ensure complete referral information

  • Verify eligibility for services including presence of supporting documentation, may use systems such as PSYCKES [Local Government Unit (LGU) level access] to assist, if necessary

  • Conduct screenings and comprehensive assessment interviews as necessary with each eligible referral, and document all related activities

  • Track and record admission decisions and/or other relevant determinations

  • Facilitate and coordinate problem-solving and communication regarding difficult to serve or ineligible referrals

  • Attend, facilitate, and/or participate in scheduled SPOE/A meetings with participating counties

  • Ensure priority handling of Assisted Outpatient Treatment (AOT) referrals; may refer hard-to-serve individuals to the Director of Community Services (DCS) for possible AOT investigation and petitions

  • Submit required reports and documentation to the Local Governmental Unit (LGU) and the Office of Mental Health

  • Coordinate warm handoffs from the Office of Mental Health (OMH) facilities to Health Home Plus Care Management or ACT as designated.

  • Determine Health Home Plus based upon SPOA designation per Department of Health guidelines.

  • Interface with family members, clients, providers, etc. to provide resources, guidance and other information re: services and supports in the community.

  • Attend monthly meetings, such as AOT/High Need, County CIT, OMH Housing Workgroup, and any other relevant meetings that may arise.

  • Attend specialty/treatment court meetings to assist with provision of services for participants, such as, attending staffing or completing screenings of individuals being referred to Mental Health/Drug/Vet Courts, etc.

    Experience, Education, & Physical Qualifications

    Any combination of education and/or experience sufficient for successful job performance will be considered qualifying. Typical qualifications may be possession of a bachelor’s degree in health or human services or an Associates’ Degree and five years’ relevant experience. Valid NYS Driver’s License required as driving is a requirement of this position.

    Knowledge:

    Demonstrated knowledge of Mental Health issues and psychiatric Rehabilitation Model; demonstrated proficiency in working directly with individuals dealing with a mental illness; demonstrated proficiency in conducting and documenting comprehensive assessment interviews; general knowledge and differential understanding of Case Management and housing programs; knowledge of relevant regulations, laws, policies and procedures.

    Skills and Abilities:

    Use of contemporary office equipment; networking with and within necessary service systems; skills in needs assessments, engaging, facilitating collaborative efforts across programs.

    Ability to communicate effectively with diverse individuals; to generate and maintain accurate documentation as required; to consistently meet deadlines, multi-task, effectively prioritize and organize time effectively, to work well under stress; to establish rapport and meaningful professional relationships; to respect and maintain appropriate confidentiality.

    Physical Requirements/Working Conditions:

    Variable with a significant amount of driving. Work may take place in client homes, hospital inpatient units and other community service environments, and general office environments.
