What you'll do Under the general supervision of Population Health division leadership, the RN Supervisor, Wellness Nurse functions in an expanded role, and is responsible for the day-to-day oversight of the Wellness Nurses. The RN Supervisor, Wellness Nurse, provides immediate supervision to all Wellness Nurses including but not limited to: daily work schedules, ensuring efficient operations and availability of wellness services for assigned locations/patients, ensures optimal care delivery through direct performance and clinical quality reviews, maintains compliance with regulatory agencies, provides network education and oversight of the Wellness Program, and guides workflow decisions and policy development for optimal delivery of care. This role is expected to provide direct patient care through completion of annual wellness visits as needed to support programmatic goals and staffing ratios. The RN Supervisor, Wellness Nurse will collaborate with clinical, financial and operational leadership in order to ensure optimal care delivery, monitoring and reporting of network level programmatic performance including budgeting and revenue generation functions. Additionally, this role will assist in developing and supporting organizational and/or division level processes and quality improvement initiatives. The Wellness Nurse Program is responsible for a full range of services related to promotion of preventive and wellness plans, illness prevention, and chronic condition management of the patient population in order to improve health outcomes. The RN Supervisor, Wellness Nurse will complete screenings, comprehensive nursing assessments, close care gaps, and provide extensive patient education. Under the direct supervision of an onsite physician, the Wellness Nurse will perform the Medicare Annual Wellness Visit for Medicare Part B beneficiaries. An individualized plan of care will be developed with the patient focused on prevention and wellness goals, risk factor modification, chronic condition self-management skills and provide linkages to other supports and services as appropriate. The Wellness Nurse will demonstrate expertise in identifying patient risk factors and will collaborate with or refer to primary care clinicians, other practitioners, and ancillary services to address acute and ongoing patient needs. The Wellness Nurse acts as a resource for the Medicare patient population, primary care, population health and quality teams. Interviews, orients and supervises Wellness Nurses Provides immediate supervision to Wellness Nurses, provides corrective action as needed, assigns work, mitigates coverage plans, evaluates performance and ensures on-going training Facilitates and oversees daily work schedules, ensuring coverage for patient care. Makes adjustments to staffing assignments and/or work locations as warranted for delivery of patient care Ensures staff completion of annual mandatory education, clinical competencies, and other applicable education to ensure staff maintain clinical nursing knowledge of disease process, risk factor identification, illness prevention, advance care planning, community resources, and nursing functions including, but not limited to, vital signs, immunizations, and collecting laboratory specimens Oversees clinical quality of Wellness Nurses including scope of practice, clinical expertise, appropriate documentation, and adherence to all required service elements. Conducts periodic observation and chart review functions Maintains knowledge of and alignment with regulatory requirements Assists manager and director with budget planning, tracking and maintaining expenses, and monitoring visit volumes performed by Wellness Nurses Assists with monitoring and evaluation of revenue capture, denied claims, and missed billing opportunities to ensure optimal financial performance and growth Assists with development, monitoring and reporting of network level programmatic performance. Oversees and engages Wellness Nurses to participate in the planning and validation of KPI’s related to Medicare Annual Wellness Visit activities and support for network or division level performance or quality improvement efforts Oversees and facilitates collaboration, workflows, and policies with clinic operations, clinicians, and other care team members to ensure patient referrals, prioritization of acute care needs, and follow up care as appropriate Ensures Wellness Nurses maintain thorough knowledge of regulatory, organizational, and/or Population Health division quality measures Performs all aspects of Medicare Annual Wellness visits including eligibility, pre-visit planning, screenings, assessments, coordination, education and prevention plan in accordance with programmatic and documentation requirements Maintains professional relationships with all internal and external colleagues Maintains confidentiality Performs other duties assigned What we need from you Education: 2 Year/Associate Degree in Nursing, required or... Vocational/Technical Diploma in Professional Nursing Program, required or... 4 Year/Bachelor's Degree in Nursing, preferred **All RN's hired and do not currently hold a BSN will be expected to demonstrate enrollment in a bachelor's prepared nursing program within 4 years of hire. BSN must be completed within 10 years. RN's licensed on or before 12/19/17 are/remain exempt from this requirement Experience: At least 5 years of relevant work experiences as an RN including supervisory and quality assurance duties.
