What you'll do The Logistics C3 is responsible for appropriate placement for patient admissions, coordinating and expediting admissions and transfers as well as overcoming delays and obstacles at Bassett Medical Center and throughout the network. Acts as a primary liaison between all service locations, support departments, other healthcare facilities, and in partnership with physicians to ensure appropriate patient placement for effective, efficient patient focused outcomes. Works collaboratively with all members of the healthcare team to improve LOS performance of the Bassett Healthcare Network. This clinical position works in conjunction with the physician and/or staff and has the authority to prioritize bed allocation resources to determine placement based on medical needs. Clinical and Technical Responsibilities. Provides centralized one-call service for direct admits and facility transfers Monitors bed requests for post-procedure admissions (ex. Cath Lab). Assigns bed and status prior to admission. Efficiently places each patient and verifies he/she is in the appropriate level of care and correct patient status/type. Uses clinical knowledge to plan and coordinate hospital-wide patient placement activities. Collaborates with Staffing Coordinator to consider nursing staff capacity in planning patient placement. Coordinates with the Unit C3, BH C3 and ED C3 to facilitate patient placement including all admissions, transfers and discharges on a 24/7 basis. Assigns Bed Request Priority. Accurately completes minimum registration tasks and obtains patient demographic information for direct admits and facility transfers in absence of C3a. Obtains clinical information and complete orders for direct admits and facility transfers. Creates and tracks direct admission and hospital transfer cases and associated documentation/activities in Intake Logistics Evaluates appropriateness of admission (patient status, level of care) for direct admits and facility transfers. Anticipates and facilitates accommodations for patient’s care needs for patients being admitted to the nursing unit (patient requiring a bariatric bed, special equipment, interpreter svc, etc.). Review previous patient history and updates CL with key patient attributes (isolation, 30-day readmission, etc.). Proactively anticipates, identifies, and addresses barriers that may impede system-wide flow; Escalates issues as appropriate. Actively leverages information to adjust resources (EVS, Transport, Ancillary Services, & Staffing) in real-time to meet Milestones and achieve length of stay goals. Appropriately escalates any cases not meeting admission criteria. Assists Logistics C3a with priorities regarding patient flow to ancillary testing and procedure areas. Serves as a consultant and educator with medical staff regarding patient status, bed placement, payer requirements, and utilization of resources. Manages Logistic Center staffing process for assigned shift as needed. Meets deadlines and exceed metrics identified for the Logistics Center through attention to detail and implementation of facility policies and Standard Operating Procedures. Follows, analyzes and reports patient placement issues, Coordinates network SIN reporting, to identify causes and trends. Responsibilities for Relationships. Works collaboratively to solve problems improve processes and integrate services within the department, with other departments and throughout the network. Makes frequent rounds in all patient care areas at Bassett Medical Center and frequent communication with clinics and affiliates to communicate with staff regarding patient placement and transfers. Performs similar or related duties as requested or directed. What we need from you Education: 4 year/Bachelor's degree in nursing or healthcare related field required or Associate degree in nursing or healthcare related field with an additional 2 years of acute care nursing experience, required All RN’s hired and do not currently hold a BSN will be expected to demonstrate enrollment in a bachelor’s prepared nursing program within 4 years of hire. BSN must be completed within 10 years. RN’s licensed on or before 12/19/17 are/remain exempt from this requirement. This position is eligible for a sign on bonus up to: USD $35,000.00 Pay Transparency Salary is based on a variety of factors, including, but not limited to, qualifications, experience, education, licenses, specialty, training, and fair market evaluation based on industry standards. Pay Range Minimum: USD $43.07/Hr. Pay Range Maximum: USD $64.60/Hr.
