Position Summary:

The Department of Family Medicine Primary Care Research Institute at the Jacobs School of Medicine and Biomedical Sciences has an exciting opportunity for a Case Manager. As a Case Manager, you will be responsible for case management and community outreach with criminal justice-involved or formerly criminal justice-involved individuals with co-occurring substance use and mental health problems in the Critical Time Intervention, using the MISSION -CJ (Maintaining Independence and Sobriety through Systems Integration, Outreach, and Networking – Criminal Justice Edition) evidence-based model.

Minimum Qualifications:

Bachelor’s degree in Psychology, Social Work or related field, in conjunction with two years of relevant experience.

Preferred Qualifications:

  • CASAC , experienced with opioid addiction.

  • Familiarity with trauma-informed care.

  • Strong case management experience
