Descriptive Title:Patient Care Associate

Full-time, Monday through Friday, 9:00am - 5:00pm, 37.5 hours per week.

Description of Duties:

-Collection of vital signs, historical medical data, caregiver/family member data. -Administration of standardized scales (training to be provided). -Scribes clinical notes - typing clinical information as visits occur, providing this information to providers for review and editing. -Sending and receiving clinical reports. -Obtaining necessary authorizations for clinical care and treatment. -Collection of data for research and grant reporting.

Qualifications:

REQUIRED: -Associates degree, BA or similar. -Experience working with older adults, experience taking vital signs required. PREFERRED: Prefer candidates with language proficiencies in Haitian Creole, Spanish, Mandarin, Cantonese, Fujianese and / or other languages commonly spoken in Central Brooklyn.
