Independence Care System Nurse Care Manager-Per Diem (Contract Role) in Brooklyn, New York

As part of the Care Management team, the Nurse Care Manager collaborates with members to build a Person-Centered Service Plan (PCSP) that is based on a comprehensive assessment and is aimed at improving and maintaining health, mobility, and community participation. This work is carried out in support of the mission and goals of Independence Care System.

Essential Functions

  1. On a annual basis, and after a significant change in condition, perform a comprehensive in-home assessment which reflects the member’s prior history, functional capacity, and stability of health—including chronic diseases/conditions, treatment plans, current providers, and the member’s identified areas of priority concerns.

  2. Develop a Person-Centered Service Plan (PCSP) with SMART interventions and goals, which is based on the assessment findings, the member’s preferences and priorities, and aims to maximize health, mobility, and community participation.

  3. Participate with Care Navigators and other team/staff members with case conferences as needed

  4. Completes documentation thoroughly and in a timely fashion.

The salary range for this position is $125.00 per Assessment.

Requirements

Minimum Qualifications

· Associate's Degree (if accompanied by extensive visiting nurse experience) or bachelor's in nursing.

· Registered Nurse currently licensed to practice in New York State.

· Experience in conducting and documenting assessment findings and recommendations.

· Highly self-motivated with the ability to manage time independently.

Additional Qualifications

· Experience with Medicaid population and/or persons with physical disabilities a plus.

· Knowledge of health care and disability issues and systems preferred.

· UAS experience (preferred)
