As a Discharge Planning Assistant, youll assist with facilitating successful discharge plans which are critical to length of stay reduction and patient flow.

Join a team of Care Management, Social Work, and Nursing, in providing a high level of customer service to patients by ensuring a proper and timely discharge. Make regular rounds of all patients to assure that the thoughtfully crafted treatment plans are being implemented and communicated across the interdisciplinary team. Go above and beyond by securing resources for patients, contacting providers and community referrals to obtain authorizations, and setting up placement after discharge.
