FEE FOR SERVICE - FIELD HOMECARE RN (All Five Boroughs)

Duties and Responsibilities

• The Field RN (Registered Nurse) performs home visits, assessments and follow up visits to patients.

• The RN advocates for quality patient care, collaborates with Physicians and other providers to maintain a safe and effective home environment for patients.

• The Registered Nurse works closely with patients, families and interdisciplinary team to ensure quality care is provided at home.

  Job Qualifications Required

• Must be a NYS Registered Nurse

• Experience working for a CHHA or Prior Hospital Med/Surg experience

• Ability to visit patients in a home environment

• Willingness to collaborate with interdisciplinary team as needed

• Excellent organizational skills required

  Education, Knowledge, Skills and Abilities Preferred

• Knowledge of ICD 10, Medicare/Medicaid and OASIS guidelines a plus

• Ability to drive and have access to reliable automobile preferred

• Bilingual Chinese, (Cantonese/Mandarin) Russian, or Spanish a plus
