  Qualifications and description for a Social Worker, LSW / CSW

• NJ Social worker, CSW / LSW License

• Conduct Social Service assesments and documents according to state and federal regulations

• provide Physco-social assesment and relevant parts of Minimum Data Set (MDS)

• Counseling Residents
