Description

Nurse Case Managers support the Pennsylvania Army National Guard (PAARNG) by managing Soldier care cases until a final disposition is made by the PAARNG.  Maintain medical and dental records in accordance with Army Regulation (AR) 40-66 while adhering to proper HIPAA laws for medical record organization and management. Soldiers requiring case management will be entered into the Case Management Program and assigned a Case Manager (CM) for tracking their status until the Soldier is either (1) considered medically ready, or (2) administratively separated from the military.  The CM will maintain ongoing contact (at least monthly) to determine and document the Soldier's prognosis. Coordinates with Soldier's unit Medical Readiness Non-Commissioned Officer (MRNCO), military medical providers, or their alternate to provide medical information concerning medical updates and counseling. Maintain medical records, both digital and paper in accordance with regulatory guidance.  Ensures accurate data entry of all Soldier medical information in appropriate medical system. Review and document clinical treatment plans from providers and transpose plans from clinical documentation into appropriate packets.  Authors proxy profiles for providers about physical restrictions and creates case summaries for providers to make critical care decisions. Maintain Health Insurance Portability and Accountability Act of 1996 (HIPAA) standards and guidelines. 2 years of clinical experience. Valid Pennsylvania State Registered Nurse (RN) or Physician Assistant (PA) License Background in case management, occupational health, and military experience is preferred. Capable of obtaining a favorable National Agency Check with Inquiries background check Proficient and professional in written and verbal communications Experience with Microsoft Word, Excel, PowerPoint, and Outlook Be familiar with Video Teleconference or Audio conferences
