Ministry Support (Diakon Lutheran Social Ministries) RN (Skilled Nursing) in Allentown, Pennsylvania

Category/Shift/Hours per 2 week pay period: Part-Time / Evening Shift / 32 hours per pay period

Luther Crest, A Diakon Lutheran Senior Living Community --- Located in Allentown with easy access to Route 78, Route 22 and Route 476. Luther Crest is a large Continuing Care Retirement Community with a skilled nursing and rehabilitation center, along with a personal care community. Luther Crest is seeking skilled and caring individuals to be part of Diakon's Many Hands One Heart team. Employees are able to use Luther Crest's state-of-the-art Wellness Center including a fitness center, spa, and indoor heated Olympic-size pool. 

General Responsibilities:

  • Responsible for managing a range of post-acute and rehabilitation patients by utilizing physical assessment skills and nursing principles. 
  • Evaluates resident's condition, gathers resident's data and analyzes obtained information to assist in the development of the resident care plan.
  • Maintains residents' electronic health record; documents observations as necessary per resident's health status and as mandated by federal and state requirements.

Education/Training: Current State license to practice as a Registered Nurse required

 

Experience: A minimum of one (1) year experience in an acute care, rehabilitation or post-acute skilled care setting preferred. Supervisory experience preferred.  

ADA: Ability to sit, stand, lift, bend, reach, push, and pull items weighing up to 50 lbs.; visual and auditory skills.
