Position Summary:

The Behavioral Health Care Coordinator is a core member of the Behavioral Health Care Management team. The functions of this role includes both direct patient care and administrative support for the Behavioral Health Child and Adolescent Program, which serves children and adolescents with behavioral health needs. Functions of this role include administrative functions, including checking patients in and out for appointments, verifying insurance, collecting co-pays, and supporting scheduling needs. There are also clinical aspects of this role, including the ability to support behavioral health crisis calls that may come in to the Wellness Center, collaboration with the patient's medical provider and behavioral health provider, and supporting patients with resources and referral needs. The Care Coordinator may also carry a small caseload using the Collaborative Care model, depending on clinic needs.

Job Responsibilities:

  • Ability to register new patients in the OOH electronic medical record (EMR) system, checking to see if the patient was previously registered and verify demographic and insurance information for accuracy.

  • Promptly and courteously, greet arriving patients and guests.

  • Promptly check patient in, including, but not limited to accurately updating insurance, yearly demographic information (registration form) and collecting any payments required from the patient. Promptly check patient out.

  • Support the mental and physical health care of patients on an assigned patient caseload. Closely coordinate care with the patient's medical providerand,when appropriate, other mental health providers.

  • Schedules future behavioral health appointments as needed.

  • Support management of behavioral health wait lists, as needed.

  • Support behavioral health crisis calls as needed, including assessing for safety risks utilizing evidenced based screening tools and follow through with patient’s medical team.

  • Provide support to patients with connection to resources in the community.

  • Provide behavioral health assessments, including depression, anxiety, and substance screenings, as appropriate, to patients.

  • Provide brief, solution focused therapeutic interventions to patients using the Collaborative Care Model, which includes using evidence-based techniques such as behavioral activation, and problem-solving treatment motivational interviewing.

  • Provide or facilitate in-clinic or outside referrals to evidence-based psychosocial treatments as clinically indicated.

  • Participate in regularly scheduled caseload consultation with the psychiatric consultation, as directed by the Behavioral Health Care Manager Supervisor.

  • Document patient calls and any therapeutic interventions into the electronic health record.

  • Any other reasonable requests from management.

Requirements

Skills/Qualifications:

  • Demonstrated ability to collaborate and communicate effectively in a team setting.

  • Ability to maintain effective and professional relationships with patient and other members of the care team.

  • Excellent time management and organizational skills.

  • Working knowledge of differential diagnosis of common mental health and/or substance abuse disorders, when appropriate.

  • Ability to effectively engage patients in a therapeutic relationship, when appropriate.

  • Ability to work with patients by telephone as well as in person.

  • Working knowledge of evidence-based psychosocial treatments and brief behavioral interventions for common mental health disorders, when appropriate e.g., motivational interviewing, problem-solving treatment, behavioral activation).

Education/Experience:

  • Experience in social work, mental health counseling or other similar work.

  • Experience with screening for common mental health and/or substance abuse disorders.

  • Experience with screening and assessment for risk.

  • Experience and training with verbal de-escalation.

  • Bachelor’s or master's degree in psychology or human services field preferred but not required.
