Trinity Health Team Lead - Care Management in Albany, New York

Description:

The Registered Professional Care Management Specialist (C3) Team Lead assumes the responsibility and accountability for collaboratively managing the progression of patient’s care with the physician and health care team utilizing evidence based best practice guidelines. The Care Management Specialist Team Lead engages the patient and care team in the joint management of key patient outcomes such as management of disease progression, continued LOS and transition to most appropriate post- acute setting. The Care Management Specialist Team Lead is a team resource who is focused on the” whole patient experience” not just the acute care episode. Ensures optimal communication, appropriate use of clinical resources and management of patient flow across the continuum. The C3 Team Lead serves as a resource and expert to the staff and provides on going education to staff, patients, and families. Nurses in this position work within and contribute to an environment where our Mission is actualized, patient focused outcomes are achieved, and high-quality professional practice is realized. This position requires excellent interpersonal communication skills that will promote team building and care coordination. The Team Lead works collaboratively with other departments to facilitate care coordination across the care continuum. The Team Lead requires collaboration with the Care Coordination Leadership team and oversite of daily case management operations.

Education Requirements :

  1. Graduate of a school of nursing.

  2. Current registration as a Registered Professional Nurse in New York State.

  3. Bachelor of Science in Nursing required, or willingness to pursue defined plan for matriculation in BSN program demonstrated by on-going course work each semester. Program must be completed within 5 years.

Experience Requirements :

  1. Three years of experience in acute care setting/one year in community health (desirable).

  2. Working knowledge of Discharge Planning and Community Resources desired

  3. Knowledge of Case Management/Utilization Management process.

  4. Certification in CPHM or CCM desired
