The Community Hospice has a wonderful opportunity for a FT RN to work as part of our Hospice Care Team caring for patients and families in local Nursing Homes.
Monday - Friday 8a-4:30p
Position Highlights:
Case managers are assigned specific nursing homes to minimize travel
NO WEEKENDS, NO ONCALL
National pre-taxed mileage reimbursement
Meetings and charting conducted via supplied lap top
Comprehensive orientation provided

What you will do:
As an RN you will case manage patients in the Nursing home setting and respond to symptom management needs of our Hospice patients and coordinate with our team to provide added support to residents and families.
Work days are Monday to Friday 8 am to 4:30 pm. NO WEEKENDS!
RN must oversee and be collaborative with the nursing home in the care of the hospice patient
What you will need:
A current license to practice as a Registered Nurse in the State of New York
Associates Degree in Nursing, BSN preferred
1 year experience as an RN required. Long term care experience highly preferred.
Previous hospice experience preferred but NOT required.
Valid Driver's License and Reliable vehicle
The RN must be able to communicate effectively, verbally and written
High level of interpersonal skills to establish and maintain relationships with patients, families, physicians, and coworkers

Pay Range: $35.00-$47.75
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
