Description

The Referral Care Coordinator serves as a vital bridge between the facility, marketing and its clientele, as well as referral sources. With a dual focus on both nurturing client relationships and ensuring a smooth, continuous flow of referral communication and follow-up.

  • Develop and maintain relationships with the existing referral sources, such as hospitals, medical professionals, therapists, and community organizations for the purpose of streamlining patient care.

  • Responsible for ensuring that coordination of care between clients and referral sources are maintained as deemed appropriate by client.

  • Act as the point of contact for referrers as it pertains to patient care.

  • Coordinate and conduct regular check-ins with referral sources, for patient updates, addressing any concerns and gathering feedback.

  • Maintain strong rapport with counselors and clientele.

  • Follow up with client's post treatment to gather feedback, testimonials, and success stories and to ensure successful transition into their next level of care.

  • Coordinate with the facility's care and administrative teams to implement suggested improvements when applicable.

  • Collaborate with the marketing team to share, develop, and implement strategies and improved systems monthly.

  • Maintain accurate and updated records of all client and referral source interactions in the facility's EMR ad CRM system.

  • Attend discharge meetings to coordinate with referral sources.

  • Assist with discharge planning and client discharge plan completion as needed.

  • Performs other duties as assigned by the Clinical Director.

Requirements

  • Bachelors Degree preferred.

  • Preferred Qualified Health Professional (QHP); CASAC, LMSW or LMHC-P in NYS.

  • Experience in client relations, case management, marketing, or outreach, preferably within a healthcare or substance facility setting is preferred.

Salary Description

$50k - $55k
